I just made one based on Stone Age Gamer's logo. If it looks a little vertically stretched in this picture, that's on purpose to account for the SNES's 4:3 aspect ratio, and it should look perfect on a TV.

I also had no issues saving on PSIV - playing through it to completion (and then some!) was one of the first things I did when I got my Mega Everdrive. I used the Japanese version with Sixfortyfive's bugfix patch, which I highly recommend, especially for the Japanese version (for the U.S. version, all it does is fix the level 99 bug).
